After your Surgery

Patient Admission & Health Questionnaire Form

We ask that we receive your completed admission forms prior to your surgery so that we can prepare for your treatment. Our clinical team will review your forms and determine if you require a pre-admission check. This may be as simple as a phone call or may require a visit to us depending on the complexity of your procedure.

If you are completing paper patient admission forms (instead of using the MyHealthcare patient portal from your specialist), please ensure they are completed and received by Grace Hospital at least two weeks before your admission date. You can submit these forms in the following ways:

In person: Please hand deliver your forms to our Reception team at Grace Hospital, 281 Cheyne Road, Tauranga Monday - Friday 7:30am to 7:30pm, Saturday 8:30am to 1:00pm.

By post: Please allow 1-2 extra weeks for post. Forms can be sent to: Grace Hospital, PO Box 2320, Tauranga 3144

By email: You can scan, photograph, or save the forms and email to admissions@gracehospital.co.nz


 
When you get home
We strongly advise you have a capable adult with you for the first 24 hours following discharge from hospital. It's important that you rest during this time so it's helpful to have additional support.

It's normal to feel some degree of pain or discomfort after an operation - your surgeon and nurses will explain what is to be expected based on your procedure.  Please also feel free to call us if you have any queries.
 
Please call your surgeon or doctor immediately if you experience
 
  • increased pain or excessive swelling around the wound
  • nausea or vomiting
  • chest pain or breathing trouble
  • fever or chills
In the event of an emergency call an ambulance (111).
